Inspector Notes
An unannounced monitoring inspection was initiated on 9/29/2025 and completed on 10/9/2025 with a local agency, in response to a self-report received by the licensing office on 9/23/2025 relating to sight and sound supervision. After review by the Licensing Administrator, the inspection documents were updated on 10/30/2025. The Department issued a Special Order on 6/17/2025 to place the licensee on probation for a period of 6 months and mandate training for all staff and any staff that supervise children. The inspector reviewed 3 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to Administration and Staff Qualifications and Training and are documented on the violation notice issued to the program.
Violations
7The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The record for Child #1, in care for 2 years, did not contain documentation of additional immunizations under the age of two years.
Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 9/23/2025, 3 children age 2 years and 3 years old, opened an unlocked gate on the playground and exited to the fenced in area behind the center. The children were out of sight and sound supervision for approximately 1-2 minutes. The center was previously cited on 8/29/2024, 10/21/2024 and 3/20/2025 for leaving a child alone and unsupervised.
Centers shall be in compliance with the center's own policies and procedures required by these standards. The center?s supervision policy is that children shall remain under supervision of staff at all times. On 9/23/2025, 3 children age 2 years and 3 years old, opened an unlocked gate on the playground and exited to the fenced in area behind the center. Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4 was missing the following required component: first date of attendance.
The following staff records shall be kept for each person. The record for Staff #1, employed for over a month, did not contain documentation of 2 references checked prior to employment. The record for Staff #2, employed for over a month, did not contain job title; date of employment; documentation of 2 references checked prior to employment; documentation to demonstrate education required by the job position.
A written agreement between the parent and the center shall be in each child's record by the first day of the child?s attendance that addresses the policy to follow regarding communicable diseases. There was no agreement in the record of Child #3, in care for 2 years, that addresses this issue.
