Inspector Notes

An unannounced inspection was initiated on 10/27/2025 and completed on 11/7/2025 in response to a complaint received by the licensing office on 10/24/2025 relating to allegations of physical abuse and neglect. The Department issued a Special Order on 6/17/2025 to place the licensee on probation for a period of 6 months and mandate training for all staff and any staff that supervise children. Two inspectors reviewed 1 staff record and 1 child?s record. This inspection included tour of the facility and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration, staff training and ratio and are documented on the violation notice issued to the program.
