Inspector Notes

An unannounced inspection was initiated on 10/27/2025 and completed on 11/7/2025 in response to a complaint received by the licensing office on 10/22/2025 relating to allegations of handwashing procedures. The Department issued a Special Order on 6/17/2025 to place the licensee on probation for a period of 6 months and mandate training for all staff and any staff that supervise children. Two inspectors reviewed 3 staff records and 1 children?s record. This inspection included tour of the facility and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration, staff training and ratio and are documented on the violation notice issued to the program.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The center is required to obtain documentation of a negative tuberculosis (TB) screening completed within the last 30 calendar days of the date of employment. Staff #3, employed for five months, TB screening was 51 days prior to the first day of employment.
