During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 11/18/2024, as a part of the licensure period. The on-site inspection began at 10:20 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 57 children present and 12 staff. The inspector reviewed 10 children?s records and 5 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/26/2024. A POC submitted after this date will not appear on the public website.

To safely perform medication administration practices, the administration shall be performed by a staff member who has satisfactorily completed a training program for this purpose.

The center had a child requiring a rectal medication, and no staff member had rectal medication administration training.
Plan of Correction: We have one MAT trained staff member. Two additional staff members will obtain certification.

Areas and equipment of the center shall be maintained in a clean, safe and operable condition.

Multiple walls in the center were seen to have chipping paint.
Plan of Correction: Areas noted will be repaired.

The required ratio for children ages 2 years old is one staff per eight children.

Staff in the two year old classroom were observed supervising 17 children (all two years old) from approximately 10:30 am to 12:45 pm.
Plan of Correction: Three of the 3 year old children in K-2 will be transition to classroom K-Prep I (K-3).
