During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on March 25, 2026. The on-site inspection began at 12:30 pm and ended at 1:40 pm. The inspector reviewed compliance in the areas listed above. There were five children present, one of which is a household member, and one caregiver; the point total was eighteen.. The inspector reviewed four children?s records and three caregiver/household members records on-site.

This inspection included document review, tour of the facility, interviews, observations, and measurements.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 8, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-570-A
A caregiver cannot exceed 16 points using a point system to determine if an additional caregiver is needed. The point system is based on ages of children in care. Provider was caring for 5 children, 1 of which is a household member under the age of 8 years of age, for a point total of 18.
Plan of Correction: Household member will be placed in childcare as soon as spot is available.
Standard 8VAC20-800-60-B
(Repeat violation) Children's records are required to contain required information. The record for Child #2 was missing a parent's employment phone number, addresses for the two emergency contacts and insurance plan number. The record for Child #4 was missing a parent's employment phone number and insurance plan number.
Plan of Correction: All information was collected and put in files for child.
Standard 8VAC20-800-690-E
Soiled disposable diapers shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper touches the exterior surface of disposal system. A soiled diaper was in a plastic bag on the floor in the bathroom.
Plan of Correction: Trash was removed immediately and placed in trash can.
