During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on June 9, 2025 and completed on June 9, 2025. The inspection started at 10:15 am and concluded at 11:00am. There were no children present and 1 caregiver. The inspector reviewed 1 child?s records and 3 caregiver/household member(s) records on-site and electronically on June 9, 2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 23, 2025.

6The vendor did not have a record of attendance.
Child 1's record did not contain: Child's full name, sex, address, and birthdate, proof of the child's identity, name, home address, and telephone number for each parent who has custody, name, address, and telephone number for each custodial parent's place of
employment or school attendance, if applicable, name, address, and telephone number of at least one person designated by
the parent to contact in case of an emergency if the parent cannot be reached, names of persons other than the custodial parent who are authorized to pick up the
child, written authorization for emergency medical care should an emergency occur
and the parent cannot be located immediately unless the parent presents a written
objection for the provision of medical treatment on religious or other grounds, Permission to transport child if the vendor provides transportation, permission for field trips, Permission for swimming or wading activities to include a parent's statement of
the child's swimming ability, if applicable, and written agreements between the parent and the vendor.
Caregiver 1's record did not document any training obtained within the previous twelve months.
There was no documentation of any shelter in place drils.l
There was no documentation that a lockdown drill had been completed.
The rabies vaccination for Dog 1 has been expired for four months.
