8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes
Inspector Notes
A Subsidy Health and Safety Inspection was conducted on 7/15/2022 from 9:40am-10:30am. There were 2 children (4 points) directly supervised by the
provider. During the inspection the home, activities and interactions with children, emergency supplies, procedures and drills were inspected.
Cleaning and sanitation routines, hand washing and diaper changing routines were also discussed. A sample of child records, provider
information and household member information was reviewed.

Violations
3documents the arrival and departure of each child in care as it occurred.
Evidence:
1. The vendor stated that there was not an attendance sheet completed during the time of the inspection.
2. There were no attendance records on file since the vendor has been in operation.
Evidence:
1. Child #1 was missing a written medication policy and authorization for emergency medical care.
2. Child #2 was missing a written medication policy and authorization for emergency medical care.
Evidence:
1. The battery-operated radio was not in working condition.
