Inspector Notes
An unannounced monitoring inspection was conducted on 07/31/2023. There were 9 children, ages 7 months- 10 years, totaling 21 points and 2 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 3 caregiver records, 1 household member records and 1 pet record were reviewed. The children were observed in free choice play, having a morning snack and in music and movement. The inspector discussed the following with the provider: infant play spaces and subsidy. The inspector arrived for the inspection at 9:55 am and departed at 11:30 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
5Evidence:
1. The most recent tuberculosis screening for Caregiver 1 was dated 06/01/2021. There was not a follow-up screening obtained before 06/01/2023.
2. The most recent tuberculosis screening for Household Member 1 was dated 05/30/2021. There was not a follow-up screening obtained before 05/30/2023.
Evidence: Caregiver 2 was observed during two diaper changes. Caregiver 2 did not wash their hands after changing each of the diapers.
Evidence: Caregiver 2 was observed during two diaper changes. The two children that were observed during diapering did not wash their hands after diapering.
Evidence: Caregiver 2 was observed changing two diapers. Caregiver 2 did not sanitize the diapering surface after each use.
Evidence: The record for Child 5, determined to be 2 years old, contained a documented start date of 02/28/2022. The most recent immunization record for Child 5 was the initial immunization record dated 02/24/2022. Additional immunization records were not obtained between 02/24/2022 and the child?s 2nd birthday.
