Inspector Notes

An unannounced monitoring inspection was conducted on 01/29/2024. There were 9 children, ages 11 months- 4 years, totaling 22 points and 2 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 3 caregiver records, 1 household member records and 1 pet record were reviewed. The children were observed in free choice play, in music and movement, circle time, during learning time and in structured center play. Infants were observed being fed on demand. The inspector discussed the following with the provider: nutrition and cleaning supplies. The inspector arrived for the inspection at 8:45 am and departed at 10:15 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the provider failed to ensure that a child?s record shall contain all of the elements as required by the standards.

Evidence: The record for Child 5 did not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there was only one designated person provided)
Plan of Correction: Will get a second contact into the record.
