Inspector Notes

An unannounced on-site monitoring inspection was conducted on 07/30/2024. The on-site inspection began at 10:45 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 14. The inspector reviewed 5 children?s records, 2 caregiver records, 1 household member records and 1 pet record. The inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the provider failed to ensure that all of the components of background checks are obtained prior to employment/residency and every 5 years there after.
