Inspector Notes

An unannounced inspection was initiated on 12/18/2024 and completed on 12/30/2024 in response to a complaint received by the licensing office on 12/10/2024 relating to background checks. The inspector reviewed 2 caregiver records on-site and electronically on 12/19/2024. The inspection included: document review; tour of the facility; interviews; and observations.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration, care of children, and nutrition and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/09/2025. A POC submitted after this date will not appear on the public website.

The provider shall ensure that when highchairs are occupied by a child, a safety strap shall be used and securely fastened.

Child 3 (9 months) was observed in a highchair without the safety strap securely fastened.

The provider shall ensure that a caregiver does not exceed 16 points by using the required point system to determine if an additional caregiver is needed.
