Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 07/17/2025. The on-site inspection began at 11:10 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 12 children and 2 caregivers; the point total was 27. The inspector reviewed 5 children?s records and 2 caregiver records on-site.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to the child?s attendance or within 30 days after the first day of attendance.

The record for Child 2, who has been enrolled for over 6 weeks, did not contain documentation of a physical examination.
Plan of Correction: Talked to the parents and have asked for a copy to be provided.
Standard 8VAC20-800-540-A
High chairs, when occupied by a child, a safety strap shall be used and securely fastened.

Child 1 (age 11 months) was in a high chair without the safety strap fastened.
Plan of Correction: Will make sure that children are strapped in.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

The record for Child 2, who has been enrolled for over 6 weeks, did not contain documentation of immunizations.
