Inspector Notes

A complaint inspection was initiated on 5/18/22 and concluded on 5/20/22. A complaint was received by the department on 3/31/22 regarding allegations in the areas of building maintenance, unlocked cleaning chemicals, ratios and staff training. An unannounced on-site inspection was conducted and interviews with staff were completed. Documentation and records related to the complaint were also reviewed. Based on information gathered during the investigation, the portion of the complaint regarding building maintenance and unlocked cleaning chemicals is determined to be valid. Violations were issued in the area of physical plant and were discussed with the center director during the exit interview.

Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. In the preschool bathroom, there is a hole at the bottom of the wall behind the toilet with an exposed water valve and piping that is accessible to the children in care. The hole also has exposed broken and crumbling drywall.
2. The center director confirmed that there is a hole in the wall of the preschool bathroom with exposed pipes and crumbling drywall.
Plan of Correction: The center responded with the following: A work order will be placed for the hole in the bathroom wall to be patched.

Based on observation, the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the two's classroom, there was a spray bottle of bleach water on an unlocked shelf.
2. The center director confirmed that there was an unlocked spray bottle of bleach water in the two's classroom.
Plan of Correction: The center responded with the following: The spray bottle of bleach water was relocated to a locked cabinet during the inspection. Center staff will be reminded to keep all hazardous substances in a locked cabinet when not in use.
