Inspector Notes
A complaint investigation was initiated on 5/8/2024 and concluded on 6/13/2024 in response to a complaints received regarding food services, an unclean bathroom and cleaning and changing of a child with wet or soiled clothing. An inspection was conducted on 5/13/2024. During the inspection interviews were conducted, and records were reviewed. The information gathered during the investigation supports the allegation, so the complaint is determined to be "valid". Violations are listed on the attached notice and were reviewed with the Director during the exit interview. If you have any questions, contact the licensing inspector at (757) 404-2322.
Violations
2Evidence:
1.During a review of the preschool bathroom on 5/13/2024 the inspector observed dirt, grime, and a dark yellow substance in the following areas.
a.There was a dried dark yellow substance under and around the hinges of the toilet seat and lid.
b.There was a dried dark yellow substance on the side and back of the porcelain base of the toilet.
c.There was a dried dark yellow substance on the floor tile at the back of the toilet.
d.There was brown dirt and grime on the left side of the porcelain toilet base.
e.There was dirt and grime on the floor on the right side, directly beside and behind the toilet tank.
2.The bolt caps to at the base of the toilet were missing. The bolts were sticking up and were accessible to children.

Evidence:
1.During nap time staff #1 stated she noticed that child #1?s clothing and linens were wet.
a. Staff could not find the child?s extra clothing.
b. Staff #1 stated the spare clothes at the center were too small for the child. The center did not have extra underwear.
c.Staff #2 notified the parent at 2:25pm that the child needed clothing due to having an accident during nap time.
d.While the child waited in wet and soiled clothing for a parent to arrive, the staff member removed the wet linens from the cot.
2. The child was not cleaned and changed until the parent arrived.
a. Staff #1 stated that when the father arrived, he took the child in the bathroom and changed her.
Teachers will be retrained to make sure soiled clothes are removed immediately and that all students have options for emergency accidents.
