During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or

Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 12/02/2024, as a part of the licensure period. The on-site inspection began at 10a.m. and ended at 11:20a.m. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 23 points. The inspector reviewed 5 children?s records and 2 caregiver and 1household member's records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3The record for Child #1 was missing the employment addresses for both parent.
The record for Child #1 and #2 was missing the first date of attendance.
The record for Child #4 was missing the employment addresses for both parents and two emergency contacts. The child was present during the inspection and had been in care for 7 months.
The record for Child #5 was missing a second emergency contact.
The provider did not clean the diapering surface with soap and water prior to sanitizing the surface.
The provider did not document the method of verifying the proof of identity in Child #1's record.
