Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 03/13/2025, as a part of the licensure period. The on-site inspection began at 11:15a.m. and ended at 12:50p.m.. The inspector reviewed compliance in the areas listed above. There were 8 children present, 1 child belonging to the provider and 2 caregivers; the point total was23 The inspector reviewed 5 children?s records and 2 caregiver's and 1 household member record on-site. This inspection included: document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
6The most recent central registry check for staff #2 was overdue by 2 months.
Children's records are required to contain parental employment addresses and phone numbers, emergency contact addresses and medical insurance policy numbers.
The record for Child #2 was missing the employment information for both parents and addresses for both emergency contacts and the medical insurance policy number. The child was present during the inspection and had been in care for 11 months.
Staff #1 did not wash their hands after diapering a child.
The diapering surface shall be cleaned and sanitized after each use.
The diaper surface was not cleaned and sanitized after Staff #1 changed a child's diaper.
An emergency evacuation procedure was not practiced during the months of January and February.
There were 2 infants that were present during the inspection that are fed with bottles. The bottles were not dated.
