Inspector Notes
An unannounced on-site complaint inspection was conducted on 12/14/2021. There were 18 children, ages 2 months-2 years, and 5 staff members present during the inspection. Information gathered during the complaint inspection determined the complaint to be Not Valid. However, non-compliance with applicable standards or law, not related to the complaint, were identified and are documented on the violation noticed issued to the facility.

Based on observation and discussion with the staff, the center failed to ensure that when personal articles are used, they shall be individually assigned.
Evidence: There were 2 pacifiers on the infant side of the facility that were not labeled to indicate individual assignment.
