Inspector Notes

An unannounced monitoring inspection was conducted on 02/24/2023. There were 21 children, ages 6 months-2years, and 9 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during diapering, handwashing, free choice activities, lunch, and naptime; infants were observed being fed and sleeping on demand. A total of 5 children?s records and 5 staff records were reviewed. The inspector arrived for the inspection at 10:00 am and departed at 12:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to obtain a copy of a child abuse and neglect search from any state in which the individual has resided in the preceding five years by the end of the 30th day of employment.

Evidence: The record for Staff 4 contained a documented hire date of 10/07/2022. The record for Staff 4 contained documentation that Staff 4 has resided outside of the state of Virginia within the preceding five years. The date on the out of state child abuse and neglect registry check was 02/01/2023.
Plan of Correction: Will get the search within 30 days of employment.
Standard 8VAC20-780-370-1
Based on observation, the center failed to ensure that when an infant is placed in their crib, they shall be placed on their back.

Evidence: Child 4 was observed being placed in their crib on their stomach.
Plan of Correction: Immediately spoke with the staff and will always place babies on their backs.
