Inspector Notes

An unannounced renewal inspection was conducted on 02/13/2024. There were 31 children, ages 2 months-2years, and 9 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during diapering, handwashing, free choice activities, and lunch; infants were observed being fed and sleeping on demand. A total of 5 children?s records, 5 staff records, and 4 applicant records were reviewed. The inspector discussed the following: safe sleep and outdoor play. The inspector arrived for the inspection at 8:45 am and departed at 12:00 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-490-E
Based on observations, the center failed to ensure that when any surface has been contaminated with body fluids, it shall be cleaned and sanitized.

Evidence: Numerous children were observed putting toys in their mouths during the inspection. The toys were observed being picked up off the floor and place back into toys bins and/or on toy shelves. The toys were not cleaned and sanitized when contaminated with body fluids.
Plan of Correction: Will create a "Yuck Bucket" to put dirty toys in for cleaning and sanitizing toys regularly during the day.
Standard 8VAC20-780-500-A
Based on observations, the center failed to ensure that staff shall wash their hands with soap and running water: before and after a diaper change; after contact with body fluids; before feeding or helping children with feeding; and before preparing or serving food or beverages.

Evidence:
1. Multiple staff were observed not washing their hands before changing diapers.
2. Staff 4 was observed wiping a child?s nose. Staff 4 did not wash their hands after contact with body fluids.
3. Staff 5 was observed giving a child a bottle. Staff 5 did not wash their hands before feeding the child.
4. Multiple staff did not wash their hands before serving the children their lunch plates.
Plan of Correction: Will remind the staff of all the handwashing requirements.
Standard 8VAC20-780-550-E
Based on document review, the center failed to ensure that shelter in place procedures shall be practiced a minimum of twice per year.

Evidence: The shelter in place procedures were only practiced once in 2023.
Plan of Correction: Will complete two shelter in place drills in 2024.
