Inspector Notes
A monitoring inspection on 04/1/2022, was conducted from 9:30am to 12pm. At the time of the inspection, 35 present children with five staff. The children were observed to be napping, playing outside, and having chapel time. Seven staff records were reviewed and five children's records were reviewed. The Program Director was able to provide a tour while LI's inspected the new area.

Based on review of seven staff records, it was determined that the facility did not ensure that staff submitted Tuberculosis Screening (TB) that had been completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #3, (DOH: 01/26/2022), had a recorded TB screening of 10/13/21.
2. Staff #6, (DOH: 02/14/2022), had a recorded TB screening of 02/14/22.
3. Staff #1, Program Director, verified the information.
Evidence:
