Inspector Notes

An unannounced inspection was conducted on January 6, 2023, from 8:55am to approximately 11:30am. There were 22 children present and 7 staff members. The Inspector reviewed 5 children?s records and 5 staff records. The Inspector observed the interaction of staff and children throughout the inspection. Children were observed during breakfast, bottle feeding, diaper changes, and indoor free play. First aid kits, medications, and emergency preparedness documents were observed. Transportation van was observed. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program. The violations were discussed with the director at the exit interview.

Standard 8VAC20-780-270-A
Based on observation and interview, it was determined that the center did not ensure that the areas and equipment of the center, inside and outside, be maintained in a safe and operable condition.
Evidence:
1. On the playground, there was a Little Tikes car that had a 5-inch crack on the hood of the car which poses as a pinching hazard to the children in care
2. On the playground, the children?s chair rocker seat had an approximately 3-inch crack between the handlebars which poses as in injury and pinching hazard to the children in care.
3. In the preschool classroom, there was a broken plastic storage bin in the free play area that was crack approximately 1 inch. This poses as a pinching and injury hazard to the children.
Plan of Correction: Plan of correction: Staff #1 threw away the chair rocker and the storage bin during the time of the inspection. The crack to the Little Tikes bike was reinforced with tape preventing the pinching hazard.

Based on observation and water temperature measurement, it was determined that the center did not ensure that a sink with running warm water does not exceed 120?F.
Evidence:
1. In the children?s bathroom, the running water temperature was measured with a thermometer and was reading 121?F.
2. Staff #1 confirmed the water heater was changed recently and the water was hot.
Plan of Correction: Plan of correction: Staff #1 will have church look at the temperature setting for the water heater this weekend and set it under 120?F.
Standard 8VAC20-780-580-C
Based on observation and interview, it was determined that the center did not ensure that during transportation of children Virginia state statutes about safety belts and child restraints are followed as required by ?? 46.2-1095 through 46.2-1100 of the Code of Virginia.
1. A child booster seat in the transportation van was expired with the date of December 2012.
2. Staff #1 confirmed the child booster seat was expired.
Plan of Correction: Plan of correction: Staff immediately disposed of the child booster seat that was expired during the time of the inspection.
