(Repeat violation) The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. 1. Peeling paint was observed in areas throughout the center, accessible to children. In the infant I room, peeling paint was observed in the windowsill. In the infant II room, peeling paint was observed in the windowsill. In the blue room peeling paint was observed in the windowsills. 2. The center?s bus used to transport children had an area in the back of a seat that was ripped and torn. 3. In the infant II classroom, there was a infant mattress inside a crib that was torn and exposing the inside material.
Plan of Correction: The leadership team will review our internal maintenance procedures with all staff to reinforce consistent monitoring and timely resolution of facility concerns. The damaged seat on the center bus was repaired on 2/18/26. The mattress pad referenced was not in use at the time of the inspection, as children have transitioned to cots; however, it has been removed and replaced. Staff have been reminded that any torn or damaged mattresses or changing pads must be removed from the classroom immediately and replaced.

Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff changed a child?s diaper in the infant I classroom and did not wash the child's hands with soap and water.
Plan of Correction: Upon notification of the violation, the staff member was immediately reminded of proper diapering procedures, including the requirement that children?s hands must be washed with soap and running water after each diaper change. The procedure was reviewed the same day with all Infant I classroom staff to ensure understanding and compliance. It was also used as a training tool at our mandatory staff training on 2.16.26.

A diapering procedure chart will be posted clearly in the diapering area at the child's and staff's eye level as a visual reminder.
