Inspector Notes

An unannounced focused inspection was conducted on 8/4/22 from 3:50pm to 5:30pm with the Director. There were 35 children in care, ranging in age from 15-months to 10-years-old, supervised by 9 staff. The children were observed having free play with the teacher, playing with legos and doing worksheets, and having free play in centers. Six child records and five staff records were reviewed. The Director reported that there are no new medications since the last inspection. The attendance was reviewed in each classroom. Based on review of six child records, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child A contained documentation that the child had a diagnosed food allergy to fish. The record for Child A did not contain an allergy care plan from the doctor.
Plan of Correction: Allergy action form is sent and the action plan is discussed with the parent.
