Inspector Notes

An unannounced complaint investigation was initiated on 7/20/22 from 9:15am to 11:25am in response to a complaint received in the licensing office regarding an allegation of a forbidden action-striking a child in care. Based on a review of documentation and interviews conducted, the allegation was determined to be valid. An exit interview was conducted with the Director on 8/19/22. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for the violation cited and return to me by close of business on 8/31/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Based on documentation review and interviews, a staff member engaged in forbidden actions by roughly handling a child and striking a child with a sandal-like shoe. Evidence: Interviews revealed that at approximately 12:30pm on 7/14/22, Child A, nine-years-old, began telling inappropriate jokes and running around the School Age classroom. Staff #1 told the child to stop and began pretending to record Child A on their cell phone, telling the child they were going to call their parent. Child A began trying to grab the cell phone and Staff #1 then spanked Child A one time on the bottom with another child?s sandal-like shoe. Child A began to yell and attempted to grab Staff #1?s cell phone again. Staff #2 overheard and came to the classroom. As Staff #2 entered the classroom, they reported witnessing Staff #1 ?aggressively push? Child A.
Plan of Correction: 1. The action was taken immediately by the Director. Staff member was suspended and no longer works in the school.

2. Director is assigning trainings to staff members that can cover topics i.e.: 1. Communicating with children, 2. Interactions matter, 3. Positive child interactions.
