Inspector Notes

An unannounced complaint investigation was conducted on 9/6/22 from 10:15am to 11:30am with the Director in response to a complaint received in the licensing office regarding allegations of injuries not being documented and staff communication. Two classrooms were observed, documentation was reviewed, and interviews were conducted. There was insufficient evidence to support the allegations and no violations were cited as a result of the investigation. One violation that was not related to the allegations was found during the inspection. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for the violation cited and return to me by close of business on 9/9/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Based on observation, the center did not ensure that the required amount of resilient surfacing was under playground climbing equipment in order to create a sufficient fall zone. Evidence: On 9/6/22 the ?Ducks'' class (two-, three-, and four-year-olds) were observed playing on a yellow school bus climber on the playground. There was approximately two inches of rubber mulch under/around the climber, with grass showing through the mulch. The requirement for a sufficient fall zone is at least six inches of rubber mulch.
Plan of Correction: Rubber mulch is ordered and would be filled throughout the playground to meet the requirement.
