Inspector Notes

An unannounced monitoring inspection was conducted on 9/7/2022. There were 21 children, ages 4 months-2 years and 5 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medication was observed onsite. A total of 4 children?s records, 3 staff records and 2 medications were reviewed. The children were observed during circle time, nap time and lunch. The inspector arrived for the inspection at 11:00am and departed the center at 1:15pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of three staff?s records, the center failed to ensure that all records were complete.
Evidence: Staff #2 sworn statement was dated 9/1/2017 and on the day of the inspection did not have the required 5 year repeat check.
Plan of Correction: Provider will have staff complete sworn statement.
