Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/12/2024 and completed on 9/12/2024, as a part of the licensure period. The on-site inspection began at 10:09AM and ended at 12:23PM. The inspector reviewed compliance in the areas listed above. There were 88 children present and 24 staff. The inspector reviewed 4 children?s records and 3 staff records on-site. This inspection included ? document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening and the documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician. The tuberculosis screening for Staff 1 was dated 4months prior to employment and was not signed by a physician during the inspection on 9/12/2024.
Plan of Correction: Provider will schedule staff for a current tuberculosis screening.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff 1 did not have orientation training within seven days of the date of assuming job responsibilities during the inspection on 9/12/2024.
Plan of Correction: Provider will ensure staff receive orientation training within 7 days of start of employment.
