Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was initiated on 5/5/2021 and concluded on 5/12/2021. The director was contacted by email to initiate the inspection. There were 13 children present and four staff. The inspector emailed the director a list of items required to complete the inspection.
The Inspector reviewed one staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined a non-compliance with applicable code sections and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on review of one staff record, it was determined that the facility did not obtain a sex offender registry check from any state where the individual has lived in the past five years prior to employment.

Evidence:
1. The sworn statement or affirmation for staff #1 indicated that she had lived in North Carolina within the past five years.
2. There was no sex offender registry check from North Carolina available for review.
Plan of Correction: All persons who have lived outside of the state of Virginia within the past five years will have appropriate background checks completed
