Inspector Notes
An unannounced complaint inspection was conducted on 01/23/2023 and 01/27/2023 in response to a complaint received regarding injury documentation, supervision and care of children.
Based on the information gathered the allegations regarding supervision and care of children could not be substantiated therefore, this part of the complaint is found to be not valid.
The allegation regarding injury documentation is found to be valid and this violation is listed on the violation notice issued to the program.

Based on record review and interview, the vendor failed to ensure that the written record of children's minor injuries shall contain all of the required elements.
Evidence:
1. The written injury report for child 1 only listed 1 of 2 injuries the child received while in care on 01/19/2023.
2. Administrative staff and teaching staff confirmed that the second injury was not included on the written injury report.
Evidence:

Plan of Correction: Administrative staff stated that the parent of child 1 was verbally notified of both injuries. Going forward, all injuries will be included on injury reports.
