Inspector Notes

An unannounced, on-site, Code Compliance inspection was initiated on 07/26/2024 and completed on 07/26/2024. The on-site inspection began at 9:50 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were 21 children present and 4 staff supervising. The inspector reviewed 4 staff records and 4 children's records on-site.

This inspection included:

*Tour of the facility;
*Interviews;
*Observations.

Information gathered during the inspection determined non-compliance with applicable law and violations are documented on the violation notice issued to the program.

Based on record review and interview, the religiously sponsored child day program failed to obtain within 30 days of employment a search of the central registry.

Evidence:
1. Staff 1, hire date 08/2021, lacks the results of a central registry check.
2. Administrative staff stated she could not find the results of a central registry check for staff 1.
Plan of Correction: Administrative staff stated that a central registry check had been obtained for staff 1 however, the results can not be located as they are stored in an email account for the previous director - which can no longer be accessed.
A new central registry check will be requested for staff 1.
