This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
A complaint inspection was initiated on 12/03/2021. A complaint was received by the department regarding allegations in the area of injury reporting. The director was contacted by telephone to conduct the investigation and injury reports were submitted.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
