Inspector Notes

A renewal inspection was conducted on April 23, 2024 with the center director and staff. There were 78 children present, ranging in age from 2 months to 5 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 9 child records and 8 staff records were reviewed. The children were having lunch and rest time.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
Based on review of nine children?s records, the facility failed to have the immunization record for each child before the child can attend the center. Evidence: Child 5 (date of enrollment 4/22/2024) and Child 7 (date of enrollment 4/8/2024) did not have documentation of an immunization record on file.
Plan of Correction: Center staff will request immunizations prior to enrollment for children.
Standard 8VAC20-780-140-A
Based on review of nine children?s records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 9 (date of enrollment 5/22/2023) did not have documentation of a physical on file.
Plan of Correction: Center staff will request documentation of a physical within 30 days of enrollment.
