At least every two years from the date of the first initial screening or testing, staff members shall submit the results of a follow-up tuberculosis screening. Staff 3 did not have a current tuberculosis test or screening; the documented test expired 3 months ago.
Plan of Correction: TB test was conducted 11/14/2025. Repeat TB tests/screenings will be repeated as needed.

Staff members caring for any child for whom emergency medications have been prescribed must have the appropriate Medication Administration Training. Child 5 has an emergency rectal medication on site, but there are no staff certified in Medication Administration Training - Rectal Medications.

At approximately 2:00 p.m. on 11/10/2025, there were 6 children present in the infant room, but 7 were marked present on the attendance record.
