Inspector Notes

A monitoring inspection was initiated on April 18, 2023 and concluded on April 19 2023. There were twenty-six children present, ranging in ages from two years to five years, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on review, facility did not ensure out of state background checks are completed when a staff has lived outside of the state of Virginia within the past 5 years.

Evidence: The record for staff #1 (start date 3/6/2023) had documentation of living in two additional states within the past 5 years and there were not completed background checks for those states to include criminal history search and child abuse/neglect search.
Plan of Correction: The background check forms will all be completed and submitted by tomorrow.
Standard 8VAC20-780-330-B
Based on observation, facility did not ensure there is resilient surfacing in required fall zones under and around climbing equipment or equipment with moving parts on the outside playground.

Evidence: The following was observed on the outside playground:
1. There is 2' of fall zone between the plastic cube slide and the wooden border of the playground when there should be at least 6'.
2. There is 4' of fall zone between the first climbing caterpillar and the wooden border of the playground when there should be at least 6'.
3. There is 3' of fall zone between a second plastic climbing caterpillar and the wooden border of the playground when there should be at least 6'.
4. There is 4' of fall zone between the plastic slide and the brick building when there should be at least 6'.
Plan of Correction: The slides and caterpillars will be re-arranged to make sure there is at least 6' around the exterior of each piece. If they cannot all fit on the playground appropriately, it will be removed.
Standard 8VAC20-780-450-A
Based on observation, facility did not ensure during designated rest period children have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on all three edges.

Evidence: The following was observed during naptime:
1. In the 2's room, 3 of 7 children did not have a bottom cover.
2. In the 3's room, 4 of 9 children did not have a bottom cover.
Plan of Correction: Staff will talk to parents about brining sleeping bags or larger blankets.

Based on observation, facility did not ensure that when pillows are used that they are assigned for individual use and covered with pillow cases.

Evidence: In the 3's classroom, 3 children were observed napping on pillows that were not covered with a pillow case.
Plan of Correction: Staff will not allow the children to nap with these pillows.

Based on review, facility did not ensure 2 of 5 children's records have documentation of required information.

Evidence: The following was found during record review:
1. The record for child #3 did not have first date of attendance and a second emergency contact.
2. The record for child #5 (start date 11/3/2022) did not have an address for one of the emergency contacts and physician information (name and phone number).
Plan of Correction: Parents will be told today that they need to fill in the information tomorrow.

Based on review, facility did not ensure staff record contains required information.

Evidence: The record for staff #1 (start date 3/6/2023) did not have a second reference check.
Plan of Correction: Staff will be told to supply a second reference letter.
