Inspector Notes
A monitoring inspection was initiated on April 18, 2023 and concluded on April 19 2023. There were twenty-six children present, ranging in ages from two years to five years, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
6Evidence: The record for staff #1 (start date 3/6/2023) had documentation of living in two additional states within the past 5 years and there were not completed background checks for those states to include criminal history search and child abuse/neglect search.
Evidence: The following was observed on the outside playground:
1. There is 2' of fall zone between the plastic cube slide and the wooden border of the playground when there should be at least 6'.
2. There is 4' of fall zone between the first climbing caterpillar and the wooden border of the playground when there should be at least 6'.
3. There is 3' of fall zone between a second plastic climbing caterpillar and the wooden border of the playground when there should be at least 6'.
4. There is 4' of fall zone between the plastic slide and the brick building when there should be at least 6'.
Evidence: The following was observed during naptime:
1. In the 2's room, 3 of 7 children did not have a bottom cover.
2. In the 3's room, 4 of 9 children did not have a bottom cover.
Evidence: In the 3's classroom, 3 children were observed napping on pillows that were not covered with a pillow case.
Evidence: The following was found during record review:
1. The record for child #3 did not have first date of attendance and a second emergency contact.
2. The record for child #5 (start date 11/3/2022) did not have an address for one of the emergency contacts and physician information (name and phone number).
Evidence: The record for staff #1 (start date 3/6/2023) did not have a second reference check.
