Inspector Notes

An unannounced renewal inspection was conducted on April 29, 2024 from approximately 10:50 am to 12:30 pm. Thirteen staff were present supervising twenty-eight children ranging in age from two through five years old. An additional staff was present to prepare lunch. Children were observed playing outside, eating lunch and laying down for nap time. Four child and four staff records were reviewed. Violation found is noted on the violation notice.

Based on review and observation, the facility did not ensure 1 of 4 staff have documentation of a negative TB test or screening within the past two years.

Evidence: The record for staff #5 had a most recent TB screening dated 9/17/2021. Staff was observed present during inspection preparing lunch for the children.
