A monitoring inspection was conducted on 08/21/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center did not have all emergency maps posted in the center.
Evidence:
The center's emergency map that was posted near the front door, did not include the shelter-in-place route.
Plan of Correction: All maps and plans will be updated to include shelter-in-place routes and procedures.
