During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/24/25. The on-site inspection began at 11:45 am and ended at 2:15pm The inspector reviewed compliance in the areas listed above. There were 78 children present and 12 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/9/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-510-E
The center's procedures for administering medication shall Include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the- counter medication may be allowed with written authorization from the child's physician and parent.

1. The medication authorization form used by the center did not identify the center as having the authority to administer a prescribed medication for child A.
2. There was no parental authorization for two medications for child B.
Plan of Correction: New authorization forms were given to parents to include the Goddard school as the authority to administer- 5/23/25.
Parents were asked to sign the two forms- 5/9/25.
Standard 8VAC20-780-510-I
The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the center administers only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container.

A prescribed medication for child A was not in the original labeled prescription container.
Plan of Correction: Parents were asked to provide the original packaging, they ordered a new medication.
