Inspector Notes

A monitoring inspection was conducted today between 9:00am and 10:30am. There were 8 children present, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed building with blocks, playing on a slide, and interacting with each other. A total of 4 child records and 2 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of 4 children's records, the family day home did not obtain documentation of additional immunizations every six months for a child who is under the age of two.
Evidence
Child #1 dob 9/16/21, does not have additional immunization records in their file, record in file was dated 3/16/22.
Child #2 dob 6/14/21, does not have additional immunization records in their file, record in file was dated 9/20/22.
Plan of Correction: I will follow up with the parents and get the updated immunization records. I will make sure to review all records and get the required documents as needed.
