Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/15/2025 and completed on 5/15/2025, as a part of the licensure period. The on-site inspection began at 2:00PM and ended at 2:48PM. The inspector reviewed compliance in the areas listed above. Medications were reviewed during the inspection. There were 5 children present and 2 staff. The inspector reviewed 5 children?s records on site on 5/15/2025 and 2 staff records electronically 4/17/2025.
This inspection included - document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Hazardous substances must be in a locked place that prevents access by children. Two bottles of cleaning solution with labels stating, ?Keep out of reach of children,? were unlocked in the cafeteria, within reach of children.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, Child #1 (age 5 years) was unsupervised for approximately 4 minutes.
