Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interview, and a virtual tour of the program.

A subsidy health and safety inspection (SHSI) was initiated and concluded on October 20. 2021. There were 3 children present with the Vendor present and supervising. Emergency plan, Emergency Drills, Attendance records, children's paperwork, and the Vendors Health Form were reviewed.

Based on interview and record review, it was determined the Vendor has not completed annual training hours.
Evidence:
1. Vendor has documentation of 4 hours in annual training but does not have the Health and Safety annual training.
Plan of Correction: I will complete and look for training hours on the DOE website.
