Inspector Notes

An announced 30-day move and annual subsidy health and safety inspection was conducted on 10/10/2023, beginning at 11:35 am and ending at 12:20 pm. There were 2 children and 1 direct caregiver in attendance at the time of the inspection. The Inspector reviewed the required background searches, training certificates, physical plant, 1 caregiver records, 2 houshold records, and 5 children's records. The children were observed playing with toys. Areas of non-compliance are identified in the Violation Notice. Based on record review and interview, before a child's first day of attendance, the vendor did not obtain documentation that a child had been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
