Inspector Notes
A renewal inspection was conducted on 05/19/2022, from 12:30pm to 2:20pm. There were 30 children present and nine staff. The Inspectors reviewed five children?s records and four staff records. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during circle time, free play, naptime, and snack. Handwashing was observed. First aid kits and emergency preparedness documents were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the program director at the exit interview.
Violations
3Evidence:
1) The record for Staff #2, (date of hire 03/24/2022) did not contain documentation of a completed criminal record check and child abuse and neglect registry check from outside of the state of Virginia. The sworn statement, signed 3/01/2022, indicated by Staff #2 had previously lived outside of the state of Virginia within the last five years.
2) The record for Staff #3, (date of hire 03/28/2022) did not contain documentation of a completed criminal record check and child abuse and neglect registry check from outside of the state of Virginia within the last five years. The sworn statement signed 3/21/2022, by Staff #3, indicated that they had previously lived outside of the state of Virginia within the last five years.
3) Staff #1 confirmed the required out of state background checks were not obtained for the staff records of Staff #2 and Staff #3.
Evidence:
1. The Little Tike Bicycle in the play area has a cracked seat approximately 2 inches in length which poses a pinching hazard.
2. The plastic play table in the playground area has a 2 inch crack to the corner of the table top.
3. The center?s owner, Staff #1, confirmed that the children use both the Little Tike Bicycle and the plastic table while in the play area and acknowledged there were cracks in the equipment.
Evidence:
1. Two of the three landlines did not have emergency numbers posted and visible at each telephone.
2. The center?s owner, Staff #1, confirmed that the emergency numbers were not posted in a visible place at each telephone.
