Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 8, 2025. The on-site inspection began at 10am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 47 children present and 13 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. A play apparatus in the outdoor playground had an approximate 16? by 3? crack to the flooring which poses as a tripping and injury hazard to the children in care. Children were observed playing on this apparatus.
Plan of Correction: The center responded with the following: The owner will have the play apparatus removed off the outdoor playground until it is safe for children to play on. The staff will purchase new flooring for this play apparatus.
