Inspector Notes

An unannounced complaint inspection was initiated by a local agency representatives and inspector on May 30,2023 and completed on August 31, 2023 regarding concerns of staffing and supervision and programs. During the inspection the following were completed: interviews with administrative staff, a review of records/documentation, and a review of video footage.

The preponderance of the evidence gathered during the investigation does support the allegation related to programs; therefore, the complaint is determined to be valid. The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff interview and documentation, the licensee did not ensure that following actions or threats are forbidden: Punishment by another child.

Evidence:
During interview Staff #1 said child #1 was poking child #2 in eye. Staff #1 said she told child #2 to poke child #1 back.

Center documentation on employee action form confirmed on May 12, 2023 Staff #1 told child #2 to poke child #1 back.
Plan of Correction: Staff #1 was terminated . Staff was coached and appropriate discipline methods an resources to use.
