Inspector Notes
An unannounced monitoring inspection was conducted on September 5, 2023 from 11:30am-2:00 pm. At the time of entrance, 32 children were in care with 6 staff members present. Additional staff was present to assist. The sample size consisted of 3 children?s records and 3 staff records. Children were
observed during nap time and in the infant room afternoon activities. Two medication were and documentation was reviewed.
Areas of non-compliance are identified on the violation notice.

The center was unable to provide documentation of Staff #1 Central registry. Staff #1 hire date in December 2022.
Based on record review and staff interview, the licensee did not ensure children have physical before first date of attendance or with in 30 days from first date of attendance.
Evidence:The center was unable to provide a physical for Child #3 start date was 12/2022.
Based on record review and staff interview, the licensee did not ensure the Virginia Department of Education-sponsored orientation course is completed within 90 calendar days of employment.
Evidence: Staff #1 date of employment 12/2022 did not have documentation of completing the Virginia Department of Education-sponsored orientation course within 90 calendar.

The most recent health inspection was August 22, 2022.
Evidence:
There were two cribs occupied during the inspection but was not identified for use by a specific child. Staff confirmed .
Based on observation, the licensee did not ensure the diapering surface is nonabsorbent surface for diapering or changing .
Evidence:
In the two and three year old room, there was a tear in the diapering surface.
Based on interview with staff and document review, the center did not ensure there were center's procedure for administering medication including methods to prevent use of outdated medication.
Evidence: During inspection Staff # 5 was unable to provide written procedures for methods to prevent use of outdated medication. Staff #5 stated the center was working on the procedures.
Evidence:
Child #1 written medication authorization form expired on 06/16/2023 . Medication was administered 07/06/2023, 07/12/2023, 07/24/2023, and 07/26/2023.

Staff #2 was in the Pre-K/Preschool classroom during nap time alone.
Evidence:
In the infant classroom, there was a child in a high chair not with a protective belt. Confirmed with staff.
Based on record review, the licensee did not ensure all staff records contained al required documents.
Evidence: Staff #1 record did not have documentation of orientation training being completed.
