Inspector Notes
The licensing inspector conducted an unannounced inspection on March 29, 2023 in response to a complaint received by the department on 03/06/2023 for allegations regarding ratios and supervision. The information gathered during the investigation supports the allegation, so the complaint is determined to be valid, and violations were documented on the violation notice issued to the facility.

Based on observations and interviews, the program failed to ensure that the required staff-to-children ratios were met.
Evidence:
1. On March 29, 2023, at 8:40 am, 13 children where under the supervision of Staff #1 in the Pre K classroom (4 and 5 years old). The required staff-to-children ratio is 1:10.
2. Staff #1 confirmed there were 13 children in the classroom.
3. Staff #2 joined classroom within three minutes.
