The licensing inspector initiated an unannounced inspection on 02/20/2024 and concluded on 02/20/2024 in response to a complaint received by the department on 02/13/24 for allegations regarding ratios. Interviews, record reviews and policy reviews were conducted relating to the allegations. The information gathered during the investigation support the allegation, so the complaint could not be substantiated.
