Inspector Notes

A subsidy health and safety (SHSI) inspection were initiated on 03/19/2024 and concluded on 03/19/2024 from 12:40 PM to 4:00 PM. There were 88 children present with 14 staff supervising ranging in age from infant to five years. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Based on record review, the center failed to require staff who work directly with children to complete the department's health and safety update course annually.

Evidence:

Staff 1, hire date 9/22, last completed the department's health and safety update course on 10/4/22.
Staff 2, hire date 3/15/23, last completed the department's health and safety update course on 11/9/22.
Staff 4, hire date 8/22/22, last completed the department's health and safety update course on 8/10/22.
Plan of Correction: Each staff member will be required to complete the department's health and safety update course. In the future the director will ensure staff complete the course annually.
