Inspector Notes

An unannounced monitoring inspection was initiated on 09/12/2024 and completed on 09/24/2024 in response to a self-report received by the licensing office on 09/03/2024 relating to sight and sound supervision. The inspector reviewed five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/09/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
There was peeling paint around the doors and windows on the toddler discovery playground.
Plan of Correction: Chipped paint to be fixed by 10/15/24.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
A two-year-old child was left unsupervised on the toddler discovery playground for approximately 35 minutes.
Plan of Correction: Staff were retrained by district leader and quality business partner on all aspects of supervision on 9/25/24. Additionally, classroom management techniques were reviewed and practiced during the training on 9/25/24. Management team to complete classroom observations weekly to ensure teachers are using best practices.
Standard 8VAC20-780-40-E
The center is to ensure that it's activities, services, and facilities are maintained in compliance with the standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Staff 1 and staff 2 did not follow procedures in completing the Child Supervision Record to ensure children were accounted for at all times.
Plan of Correction: Teachers retrained on Child Supervision Records (CSR) best practice on 9/25/24 with region quality business partner. Intentional focus on name to face transitions, maintaining ratios, signing in and out as a teacher, and all other layers of the CSR document.
