Inspector Notes

A non-mandated focused inspection was conducted on June 26, 2024 from 1:15 P.M.-3:15 P.M. The focused inspection was completed to investigate a self-report by the facility reported on June 14, 2024 regarding child abuse and neglect. The inspector reviewed compliance in the areas of administration, staffing and supervision, staff qualifications and training, programming, background checks, and child abuse and neglect. Five staff were interviewed, and one child record and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age
children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the
program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Based on a review of staff records, the center failed to ensure that a central registry finding
had been completed and on file within 30 days of employment.
Evidence:
1. Staff #1's date of hire was 05/21/2024. There was not a completed central registry finding in the staff record.
2. Administration verified the date of employment for Staff #1, and that there was not a completed central registry finding in the record.
Plan of Correction: Set up chart to check every 30 days for CRS until the facility received the completed background check. Email sent for Staff #1 to check on the CRS.
